INAIL self-assessment: how the annual premium cycle affects partner cash flows

Why the premium cycle matters to a platform

INAIL is the Italian insurance body for workplace accidents and occupational disease. Every employer pays a premium to it. For a global EOR or payroll platform with Italian workers, that premium is a real cost line. It is also a cash flow event that does not follow the monthly payroll rhythm.

If your finance team models Italy as a simple monthly cost per employee, the INAIL cycle will create gaps between forecast and actual cash. This article explains the cycle and shows where the gaps come from.

How self-assessment works

INAIL uses a self-assessment model. The employer, or the party acting for it, calculates the premium itself. INAIL does not send a bill for the amount due. The cycle has two parts that overlap.

1. Settlement of the closed year. The employer calculates the final premium for the year that has ended. This uses the actual payroll paid to insured workers during that year.

2. Advance for the current year. In the same step, the employer pays an advance on the year now running. The advance is based on an estimate of payroll for that year.

So one payment covers two things: the balance or credit from the past year and the advance for the new one. The net amount can look odd. A company that grew headcount during the year owes a larger balance. A company that shrank may see a credit that offsets the advance.

Where partner cash flows are affected

Three points matter for planning.

The payment is lumpy. Monthly payroll costs are smooth. The INAIL payment is a single annual event, with the advance and the settlement combined. Treasury should hold a reserve for it rather than discover it at the deadline.

Headcount changes move the settlement. Hiring during the year raises actual payroll above the estimate used for the advance. The gap comes back as a balance due at settlement. Terminations work the other way. If you onboard many workers for a client in the second half of the year, expect a larger settlement.

Client recharge timing is a choice. You can recharge the premium to the client monthly as an accrual, or you can recharge it when the annual payment is made. The first option smooths your cash. The second option leaves you carrying the cost for months. Your service agreement should say which one applies.

What changed in 2026

Trade press reports that the 2019 rate tariffs (DI 27/02/2019) remain in force. What changed is how they are applied. A ministerial decree, DM 02/04/2026, published on 03/06/2026, modified the application rules for the rate reduction known as oscillazione.

Two points from the reporting are relevant to partners:

  • There is a new Tabella A Bonus. It is based on the number of worker-years (lavoratori-anno) and on the ISAR indicator.
  • The fixed reduction for non-significant PAT positions is now 13%. It was 5%.

A PAT is the insurance position that INAIL holds for an employer. Where a position is treated as non-significant, the reduction is now fixed at the higher rate. The reduction applies to the premium, so the settlement payment can come out lower than in earlier cycles.

These facts come from secondary reporting. Check the decree text and INAIL guidance before you change a client price list. Your Italian payroll provider should confirm how the new rules apply to each client’s position.

Practical consequences

The lower reduction threshold is good news for cost, but it adds modelling work.

  • Do not assume the same reduction for every client. The treatment depends on the position and on its significance. Two clients with similar headcount may see different outcomes.
  • Expect credits. A larger reduction can turn a settlement into a credit against the advance. Decide in advance whether the credit is passed to the client or netted against other charges.
  • Watch worker-years, not only headcount. The new table uses worker-years. A person employed for part of the year counts for part of a unit. Your data should record start and end dates cleanly.
  • Keep the pricing transparent. If your fee includes INAIL as a pass-through, the reduction should reach the client. If it is bundled into a fixed fee, the saving stays with you. Make that choice explicitly.

A short checklist for partnership and finance teams

  1. Map when the combined payment falls and reserve cash before it.
  2. Accrue the premium monthly in the client ledger.
  3. Log worker start and end dates in a form that supports a worker-year count.
  4. Confirm with your Italian provider how the 2026 rules apply to each position.
  5. Agree contractually how reductions and credits are shared.

None of this is complicated once it is in the model. The problem is that it is easy to miss until the first annual payment arrives.

Partnership teams that want to automate the exchange of headcount and premium data can request the data-exchange specification.
